Urgent eye symptom check

A short set of questions to separate routine booking from symptoms that should be reviewed quickly.

Information
This block does not diagnose. It helps route the patient safely to a doctor or quick clinic contact.
Instruction
Mark symptoms that are present now or appeared suddenly. If at least one urgent sign is selected, the system will show priority booking or a call option.

Did vision in one or both eyes suddenly become much worse?

Especially if it happened within hours or 1–2 days.

Is there severe eye pain, intense redness, or light sensitivity?

These symptoms should not usually wait for a routine visit.

Have you noticed flashes, many new floaters, or a curtain-like shadow?

This is a reason to contact the clinic quickly.

Was there an eye injury, burn, or chemical exposure?

This situation needs quick professional assessment.

Distance visual acuity check

A preliminary check of how clearly you see symbols at a distance.

Information
Accuracy depends on screen size, distance, lighting and calibration. This is not an eyeglass prescription.
Instruction
Stand at the recommended distance from the screen, cover one eye, and select the smallest row you can read confidently. Repeat for the other eye.

If unsure, choose the previous larger row.

Left eye: what is the smallest row you read confidently?

Cover the right eye without pressing on it.

Astigmatism sign check

A radial-line target: with astigmatism, some lines may look darker, sharper or blurrier.

Information
This is only a screening check. Astigmatism is measured accurately during an in-office refraction.
Instruction
Look at the center of the target with each eye separately. Select whether all lines look similar or certain directions stand out.

Amsler grid

Helps notice distortion, dark spots or missing areas in central vision.

Information
If lines are distorted, missing, or a dark spot appears, booking with an ophthalmologist/retina specialist is recommended.
Instruction
Wear reading glasses if you use them. Cover one eye, look at the center dot and note whether the lines are straight and complete.

Preliminary color vision check

A screen-based check inspired by pseudoisochromatic plates: the user should identify a number on a colored field.

Information
Displays, night mode and brightness can distort the result. A professional test is required for a reliable conclusion.
Instruction
Look at the image at normal screen brightness and choose the number you see. Do not change color settings during the test.

Dry eye symptoms

A questionnaire about burning, gritty sensation, fluctuating clarity and discomfort often associated with dry eye.

Information
The questionnaire does not identify the cause of dryness. The doctor may recommend tear-film tests.
Instruction
Rate how often these symptoms have bothered you recently. Answers help suggest the right consultation or diagnostic service.

Digital eye strain

Symptoms may relate to refraction, dry eye, lighting, screen position or work habits.
Instruction
Answer based on a typical working day. The result helps suggest a preventive exam, refraction check or consultation on screen-related load.
